South West Care Services provides in-home care and community access to Aged and Disability participants across Central and Greater Sydney. About the Role:
As a Community Worker, you will deliver a range of domestic, social, recreational, and personal care services to ensure participants experience quality living, social inclusion, and overall wellbeing. This is an excellent opportunity to further develop your career within a supportive and evolving organisation.
Key Responsibilities:
- Deliver domestic, social, recreational, and personal care support to participants.
- Develop, implement, and review client support plans in line with NDIS guidelines.
- Assist clients with goal setting, progress monitoring, and participation in activities.
- Advocate for clients and support access to community services.
- Build relationships with community organisations and service providers.
- Support education, reporting, and fundraising initiatives as required.
- Undertake professional development and perform other reasonable duties.
Role Requirements:
Qualification in Individual Support, Disability, Community Services or Aged Care
Current First Aid Certificate
Current NSW Working With Children Check (or willing to obtain)
Police Check (or willing to obtain)
NDIS Workers Check
